The single most preventable source of patient injury is medication errors, which include errors in prescribing, dispensing, or administration of a drug. They have a substantial impact on the efficiency of pharmacy workflow, raise patient safety issues, and place a financial burden on healthcare systems. For many years, researchers have focused on lowering the frequency of drug errors in cancer treatment, with the idea that interdisciplinary approaches are necessary to ensure continual improvement.
